出生登録の申請用紙はミャンマー大使館のウェブサイトにて入手できます。

注意：「出生届・出生証明書」は一度役所に提出して、そのあとに、市長などの印鑑が押されているものを発行してもらうこと。市長印が無いものは大使館で断られる場合もあり、弊社ではお受けできません。

ホッチキスを外すと無効になる場合がありますのでご注意ください。

We receive a large number of requests for translations related to birth registration for the Myanmar Embassy. Most commonly, clients request a set consisting of the following documents:

• Shusshotodoke Kisaijiko Shomei (Certificate of Matters Stated in the Birth Notice, Birth Notice / Birth Certificate)

• Jurishomeisho for birth notice (Certificate of Acceptance of Birth Notice)

• Jurishomeisho for the parents’ marriage notice (Certificate of Acceptance of Marriage Notice)

The application form for birth registration can be obtained from the website of the Myanmar Embassy.

Note: The “Birth Notice / Birth Certificate” must first be submitted to the municipal office, after which a certified copy bearing the mayor’s seal will be issued. Documents without the mayor’s seal may be rejected by the embassy, and therefore our company cannot accept them.

Please note that removing the staples may invalidate the document.

မွေးစာရင်းသွင်းရန် လျှောက်လွှာဖောင်ကို မြန်မာသံရုံး၏ ဝက်ဘ်ဆိုဒ်တွင် ရယူနိုင်ပါတယ်။

မှတ်ချက်။ “Shusshotodoke / Shussho Shomeisho” ကို ပထမဦးစွာ မြို့နယ်ရုံးသို့ တင်သွင်းရပြီး၊ ထို့နောက် မြို့တော်ဝန်၏ တံဆိပ်ပါသော အတည်ပြုမိတ္တူကို ထုတ်ပေးပါသည်။

မြို့တော်ဝန်၏ တံဆိပ်မပါသော စာရွက်စာတမ်းများကို သံရုံးမှ ငြင်းပယ်နိုင်သဖြင့်၊ ကျွန်ုပ်တို့ကုမ္ပဏီက လက်ခံမရနိုင်ပါ။
